Visitation Parish Sacramental Record Request Form

Visitation Parish retains the Sacramental Records of Sacred Heart Church (1905 to Present), Manchester by the Sea, Ma and St. John the Baptist Church (1931 to Present), Essex, Ma. Any person may request a Sacramental Certificate of his or her own Sacramental Record. In addition to the person who received the sacrament, the parents of a minor, one with legal guardianship of another, a Pastoral Minister, Another Catholic Parish, or a Diocesan Official may obtain an individuals Sacramental information. Requests maybe made in writing, by mail, or with this form. AS these records contain sensitive information, only Parish Staff are allowed access to the register books.
